Campbellford Memorial Hospital has announced an expanded partnership with the Toronto Grace Health Centre, aimed at providing eligible patients with remote care monitoring and personal support worker (PSW) assistance.
This initiative is designed to ensure a safe discharge home for patients awaiting long-term care, thereby alleviating the hospital’s alternate level of care pressures.
The fully-funded Remote Care Monitoring Plus program, approved in the summer of 2023, offers a comprehensive solution for patient care. The program equips patients with a pendant that utilizes geolocating technology to detect wandering or falls. Additionally, it provides extensive PSW support and access to nursing care through Home and Community Care services.
